Sections of documents that ask you to mark or draw areas of pain or symptoms may left blank until your visit unless your computer has editing capabilities. Still complete the rest of the document.

 Once you complete the forms, go to file tab, then select down menu to PRINT, select PRINT AS PDF to ensure a copy of the PDF with your filled information is saved. Save a copy to your computer.

 

You can then attach the saved file to an email or upload it to your patient portal directly if you are a member.
