DPC Memberships
Individuals:
0-17 yo $50/mo w/adult
18-64 yo $100/ month
65 and over $120/mo
Membership (separate from procedural treatments like OMT, acupuncture) includes:

Initial comprehensive in person consultation, up to 4 visits a month

Direct to doctor full text, phone, video access using Spruce/Elation

Low-cost point of care testing (strep/urinalysis/pregnancy test, more)

Access to low-cost meds, labs, diagnostics, office procedures. Fees apply.

Additional Visits:

$200 house visit within 3 square miles of office, + 2$ per mile to and from visit. Max distance to discretion of provider and subject to additional fee. House visit fee does not include procedures, meds, labs or other possibly necessary items.
You may use insurance for meds, labs, diagnostics at facilities that accept your insurance; however, we are not responsible for any charges that are not covered or disclosed by your insurance. It is up to you to verify that something is covered before you choose to use your insurance.
We DO NOT accept insurance or process authorizations or claims.
We can help you navigate the system if needed.
Paperwork administration fee of $25 per page applies on all forms, and letters.
For less than the average monthly cell phone bill, get one on one attention with your doctor when you need it.
$75 one time registration fee per member
$200 Re-registration fee if prior membership lapse or cancellation

*Note: 2.5% credit card transaction fee + .50 cent will be charged to credit charges; additional 1% surcharge added to American Express transactions.
OMT/OMM Treatment
Fee includes consultation, treatment, and self care planning. Level of visit is calculated based on number of areas treated, time spent and complexity. OMT membership is not available at this time.
Fee is separate service charge and does include cost of added services like injections, house call fee, medications, etc.
Average DPC member price per session: $135. You must already be an establish DPC patient to access special pricing.
