DPC Memberships
Individuals:
0-17 yo $50/mo w/adult membership
18-64 yo $100/ month
65 and over $120/month
Auto billed monthly.
First month and registration fee of $75 will be billed prior to first visit.

HOUSE VISITS

Within 2 square miles of office: $200 base charge; add $5 per mile outside 2 miles to and from visit. Max distance to discretion of provider and subject to additional fee. House visit fee does not include procedures, meds, labs or other possibly necessary treatments.
You may use insurance for meds, labs, diagnostics at facilities that accept your insurance; however, we are not responsible for any charges that are not covered or disclosed by your insurance. It is up to you to verify that something is covered before you choose to use your insurance.
We DO NOT accept insurance or process authorizations or claims.
We can help you navigate the system if needed.
Paperwork administration fee of $25 per page applies on all forms, and letters for any documents needing completion from your employer, insurance, etc and is to the discretion of your treating doctor.
For less than the average monthly cell phone bill, get one on one attention with your doctor when you need it.
$75 one time registration fee per member
$200 Re-registration fee if prior membership lapse or cancellation

*Note: 2.5% credit card transaction fee + .50 cent will be charged to credit charges; additional 1% surcharge added to American Express transactions.
If you cancel your membership, the fee already paid for that cycle will be non refundable. Any exceptions will be at the discretion of the practice.
OMT/OMM Treatment
Fee includes consultation, treatment, and self care planning. Level of visit is calculated based on number of areas treated, time spent and complexity. OMT membership is not available at this time. OMT treatment is NOT included in the DPC member. DPC members get a special rate for OMT services.
OMT Fees are a separate service charge and does include cost of added services like injections, house call fee, medications, primary care discussion items etc.
Nonmember price per session starts at:
$375 initial consultation
$185-$275 follow up consultation based on complexity of treatment.
Patients seen for follow up beyond 3 months since last visit will be charged a higher follow up rate starting at $260.
Please be aware that if you want a Primary Care Medical Type Consultation in addition to an Osteopathic Treatment there will a separate charge for the medical consult and the Osteopathic Medical Consult.
For example: Medicine Consult for blood pressure, diabetes, or comprehensive physical $375, plus OMT treatment follow up $185.
DPC members have special price per session: $135-185 depending on complexity. You must already be an established DPC patient to access special pricing.
