DPC Memberships
0-17 yo $50/mo w/adult membership
18-64 yo $100/ month
65 and over $120/month
Auto billed monthly.
First month and registration fee of $75 will be billed prior to first visit per member.
$200 Re-registration fee if prior membership lapse or cancellation.
*Note: recommend use of bank account for payment to avoid credit card fees
*Note: 2.5% credit card transaction fee + .50 cent will be charged to credit charges; additional 1% surcharge added to American Express transactions.
If you cancel your membership, the fee already paid for that cycle will be non refundable. Any exceptions will be at the discretion of the practice.
